Instructions for your first Amatsu appointment
Please arrive early for your first visit. We would ask you to please fill in the consent form below in advance of your session. If you are unable to do so then please arrive at least 20 minutes early to process the necessary paperwork before you session.

Clinic Policy
- Clients are asked to arrive on time for all appointments, as late attendance ultimately shortens the session.
- Please give at least 24 hours notice for cancellation of appointments, otherwise you may be charged a fee.
- This is a clinical appointment and as such clients are required to be hygienic in presentation.
Consent Form
In order to prepare for your first treatment we need to collect some information about you. Rather than take up appointment time we ask you to fill this out in advance. You can do so online or print it out and bring it with you.
